Transitional Living Program
This teen independent living program supports teenagers ages 17-21 in the custody of the Department of Children, Youth and Families. These teens, some of whom may be returning from out-of-state placements, training schools or hospital stays, work with case managers as they develop the skills they will need to live fully independent lives.
Teens, living in their own apartments, receive intensive case management and support through involvement in clinical, vocational and educational services. Staff members work flexible hours to provide day, evening and weekend support and activities. Teen moms who participate in this program with their children receive enhanced coordinated care with the Early Childhood programs.
