PCHC At North Main

Providence Community Health Centers (PCHC) at North Main is a collaboration with The Providence Center that creates a patient-centered medical home for clients.

PCHC at North Main provides total health care for Providence Center adult clients experiencing mental health and substance use problems. Patients receive coordinated physical, mental health and substance abuse care under one roof with access to community supports that promote a healthy lifestyle. An integrated care coordinator works with clients and the PCHC and TPC teams to make it easier for them to understand and manage their physical health needs.
